Introduction Sarcomatoid carcinoma (SC) is a rare subtype of malignant neoplasm with poor prognosis that involves both carcinomatous and sarcomatous components. Although it may develop in various organs, SC the large intestine has rarely been reported. It not for patients to have distant metastasis, reflecting its highly aggressive oncologic features, but cases brain metastasis on initial visit are rare. In this report, we described case transverse colon metastases whose symptom was neurological disorder, reviewed 31 reported SC. Case presentation A 70-year-old man admitted our hospital chief complaints gait disorder severe dizziness. Head magnetic resonance imaging revealed tumor masses anterior lobe cerebellum. detected by colonoscopy abdominal enhanced computed tomography (CT), diagnosed as undifferentiated adenocarcinoma histology. Laparoscopic extended right hemicolectomy performed remove obstruction, resected specimens an invasive consisting mixture According immunopathological study, patient The clinical course extremely aggressive, died 28th postoperative day because disease progression. Conclusion To best knowledge, first derived from metastases. This experience contribute guidance regarding proper therapeutic options
